ASSUMPTION OF RISK, WAIVER, AND RELEASE OF LIABILITY

PLEASE READ CAREFULLY. THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS.

1. Participation and Age Verification I desire to participate in a bourbon tasting, workshop, tour, or event (the “Event”) hosted or organized by The Bourbon Goddess, LLC. I hereby certify that I am at least twenty-one (21) years of age. I understand that I may be required to present a valid government-issued ID to verify my age before participating.
2. Assumption of Risk I understand that the Event involves the consumption of alcohol. I acknowledge that the consumption of alcohol involves inherent risks, including but not limited to impaired judgment, impaired motor skills, and physical injury. I voluntarily assume all risks associated with my participation in the Event, whether known or unknown.
3. Responsible Consumption I agree to consume alcohol responsibly and within my personal limits. I understand that The Bourbon Goddess, LLC reserves the right to refuse service to anyone who appears intoxicated or is acting in a disruptive manner. I agree not to operate a vehicle if I am impaired and will arrange for alternative transportation (e.g., rideshare, taxi, designated driver) if necessary.
4. Release of Liability In consideration of being permitted to participate in the Event, I, for myself, my heirs, personal representatives, and assigns, do hereby release, waive, discharge, and covenant not to sue The Bourbon Goddess, LLC, its owners, officers, employees, agents, and venue partners (collectively, the “Releasees”) from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the Releasees or otherwise, while participating in the Event.
5. Indemnification I agree to indemnify and hold harmless the Releasees from any and all liabilities or claims made by other individuals or entities as a result of my actions during the Event.
6. Media Release I grant The Bourbon Goddess, LLC the right to take photographs and/or video recordings of me in connection with the Event. I authorize The Bourbon Goddess, LLC to use such images for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content, without compensation to me.
7. Governing Law This Waiver and Release shall be construed in accordance with the laws of the State of Georgia.

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT (OR BY CHECKING THE BOX INDICATING ACCEPTANCE). I SIGN IT FREELY AND VOLUNTARILY.
